On Patient Juan™’s right part of the top bar, you can see the “Form Builder” button, which contains various forms provided by Patient Juan™.

Patient Juan™ contains the following forms:

• Take Home Instructions
• Prescription
• Diagnostic Tests
• Medical Certificate
• PhilHealth Claim Form 4

Note: All forms made in the Form Builder are not recorded as a consultation. Any information placed in any of these forms will not be recorded as well in the patient’s history.
